Disciplinary Action Search
The Board may discipline an optometrist who violates the Texas Optometry Act. To find the disciplinary history of an optometrist, read the Search Policies and click on the "Agree" link below. You may also telephone the Board at 512-305-8500.
 
Date of Disciplinary Action
An optometrist with past disciplinary action may currently be able to practice without restrictions. Use this link to determine the date of the disciplinary action. If there are questions regarding the current status of a license, please call the Board at 512-305-8500.
 
Copies of Disciplinary Action
A copy of disciplinary action is available upon a written request under the Public Information Act. An official copy under the official seal of the Board is also available upon a written request. The fee for the official request is $15.00.The search results are obtained from information contained in a database maintained by a private vendor. Therefore the Texas Optometry Board cannot warrant or guarantee that each database entry was downloaded accurately.

License number, name, location, status of license, expiration date, and disciplinary action. The Optometry Board does not issue board certifications, and does not maintain such information in its database.
